In 2022, over 1.3 million users of the national health system in Portugal did not have a general practitioner (GP). This situation was particularly dramatic in the region of Lisboa e Vale do Tejo, since 946,590 patients, or 25 percent of the total number of active registered users, did not have a GP. As of 2024, the number of users without a family doctor increased to more than 1.5 million in mainland Portugal. As of March 2025, this figure was even higher.

The source adds that the data presented on the number of users without a family doctor does not reflect the entire universe of primary healthcare users, but only includes users enrolled in family health units (USFs) and personalized healthcare units (UCSPs).
