In 2023, among Medicare beneficiaries who had an inpatient hospital stay, **** percent were discharged to formal post-acute care settings including: home health agencies, inpatient rehabilitation facilities, long-term acute care hospitals, and skilled nursing facilities. The majority (just more than half) were discharged to the community (without skilled home health care), which included the individual's home, assisted living facilities, and retirement communities.

**Other: Hospice, a different inpatient hospital, or other inpatient hospitals such as inpatient psychiatric facilities.
